Cluster Headache Disease Understanding

According to the National Organization of Rare Diseases (NORD), Cluster headaches (CHs) are an uncommon, severe form of primary neurovascular headaches. CH is the most painful form of headaches, with the pain occurring on one side of the head and behind or above the eye or at the temple most commonly.


CH is subcategorized into episodic and chronic forms based on certain factors like duration, frequency, periodicity, etc. In episodic cluster headache (ECH), at least two cluster periods lasting 7 days to 1 year, separated by pain-free periods lasting 1 month or more, are seen. In chronic cluster headache (CCH), attacks occur for more than 1 year without remission or with remission lasting less than 1 month.


As reported in a recent report by Diana Yi-Ting Wei et al. (2018) in the article titled “Cluster Headache: Epidemiology, Pathophysiology, Clinical Features, and Diagnosis” gives the low prevalence of cluster headache compared to migraine, it is difficult to assess the prevalence of cluster headache in the community accurately. Nonetheless, the author suggests that, given the specific features of cluster headache, the community can be identified using questionnaires based on the ICHD criteria. The study identified seven subjects with cluster headaches, corresponding to a prevalence of 381 per 100,000. Since August 2007, there have been two further population-based studies, one from the Republic of Georgia with a prevalence of 87 per 100,000 and in rural Ethiopia with a prevalence of 1.3%. Cluster headache has been historically considered to have a male preponderance, with a high male-to-female ratio, and is thought to be different between episodic cluster headache (ECH) and chronic cluster headache (CCH). In a study involving 545 patients with ECH and CCH examined between 1963 and 1997, it was observed that there was a downward trend in male preponderance over this period. However, there is still an overall male preponderance for cluster headache.


In another report titled “The incidence and prevalence of cluster headache: a meta-analysis of population-based studies” written by Fischera et al. (2008), the pooled data showed a lifetime prevalence of 124 per 100,000 and a 1-year prevalence of 53 per 100,000. The ratio of episodic vs. chronic cluster headache was 6.0. This analysis revealed a relatively stable lifetime prevalence, which suggests that about one in 1000 people suffers from cluster headache, the prevalence being independent of the region of the population study. The sex ratio (male to female) is higher than published in several patient-based epidemiological studies.
