Acute heart failure (AHF) is a clinical syndrome characterized by the rapid onset or worsening of signs and symptoms of heart failure that requires urgent medical evaluation and treatment. It may present as de novo heart failure in patients without a previous history of HF or as acute decompensated heart failure (ADHF) in individuals with chronic HF. AHF is primarily associated with systemic and pulmonary congestion resulting from elevated cardiac filling pressures and may lead to impaired organ perfusion if left untreated. Despite advances in chronic heart failure management, AHF remains a major cause of hospitalization, with high rates of mortality and readmission.
AHF develops in the presence of an underlying structural or functional cardiac abnormality and is often precipitated by acute events. Common causes include acute coronary syndrome or myocardial infarction, worsening chronic heart failure, uncontrolled hypertension, cardiac arrhythmias (particularly atrial fibrillation), valvular heart disease, myocarditis, and cardiomyopathies. Non-cardiac precipitating factors such as respiratory infections, pulmonary embolism, renal dysfunction, anemia, medication non-adherence, excessive salt or fluid intake, and adverse drug effects can also trigger acute decompensation by increasing cardiac workload or promoting fluid retention.
The pathophysiology of AHF is heterogeneous and depends on the underlying cardiac disorder and precipitating factors. Acute impairment of systolic and/or diastolic ventricular function leads to increased ventricular filling pressures, elevated preload and afterload, and reduced cardiac output. Neurohormonal activation involving the sympathetic nervous system and the renin–angiotensin–aldosterone system initially compensates for reduced perfusion but eventually becomes maladaptive, promoting vasoconstriction, sodium and water retention, and ventricular remodeling.
The diagnosis of AHF is based on a combination of clinical assessment, laboratory investigations, and imaging studies. Patients typically present with acute dyspnea, orthopnea, pulmonary crackles, peripheral edema, elevated jugular venous pressure, and signs of congestion or hypoperfusion. Diagnostic evaluation includes electrocardiography, chest radiography, echocardiography, and measurement of natriuretic peptides (BNP or NT-proBNP) to support the diagnosis of heart failure. Cardiac troponins, renal and liver function tests, and assessment of oxygen saturation help identify precipitating factors, evaluate end-organ involvement, and guide therapeutic decision-making.
The primary goals of AHF treatment are rapid stabilization of the patient, relief of congestion, correction of hypoxia, and management of the underlying cause. Initial therapy includes oxygen supplementation in hypoxemic patients, non-invasive ventilation for respiratory distress, and intravenous loop diuretics to reduce fluid overload. Intravenous vasodilators may be administered in patients with adequate blood pressure to decrease cardiac preload and afterload, whereas inotropes and vasopressors are reserved for patients with severe hypoperfusion or cardiogenic shock. Early identification and treatment of precipitating conditions, followed by optimization of guideline-
directed heart failure therapy before discharge, are essential to improve long-term clinical outcomes and reduce the risk of rehospitalization.
