Ascites is the pathological accumulation of fluid within the peritoneal cavity and is most commonly associated with liver disease, particularly cirrhosis, although it can also occur due to cardiac, renal, pancreatic, infectious, and malignant conditions. In patients with cirrhosis, the development of ascites often indicates progression from compensated to decompensated liver disease and is associated with complications such as spontaneous bacterial peritonitis and hepatorenal syndrome.

Cirrhosis is the predominant cause of ascites, accounting for approximately 80% of cases in the United States, with alcohol-related liver disease, metabolic dysfunction-associated steatotic liver disease (MASLD), and chronic viral hepatitis being major underlying causes. Other causes include peritoneal malignancy, congestive heart failure, nephrotic syndrome, tuberculosis, pancreatitis, pancreatic duct disruption, and lymphatic disorders causing chylous ascites. In cirrhotic ascites, portal hypertension is the initiating event, resulting in increased nitric oxide production and splanchnic arterial vasodilation. This reduces the effective arterial blood volume and activates the renin-angiotensin-aldosterone system, sympathetic nervous system, and arginine vasopressin, promoting renal sodium and water retention. Increased splanchnic capillary pressure and lymph production further contribute to fluid accumulation in the peritoneal cavity.

Diagnosis involves clinical examination, imaging, and analysis of ascitic fluid. Patients commonly present with progressive abdominal distension, discomfort, weight gain, early satiety, and dyspnea, with physical findings such as shifting dullness and a fluid wave. Diagnostic abdominal paracentesis is the key initial investigation, with ascitic fluid analyzed for cell count, culture, cytology, albumin, and other parameters. The serum-ascites albumin gradient (SAAG) helps determine the underlying cause: a SAAG ?1.1 g/dL suggests portal hypertension, whereas a SAAG <1.1 g/dL suggests non-portal hypertensive causes. Ultrasound is highly sensitive for detecting ascitic fluid, while CT can help identify underlying masses or other pathology.

Management is directed toward the underlying cause and reduction of fluid accumulation while maintaining adequate intravascular volume. For cirrhotic ascites, dietary sodium restriction and diuretics, particularly spironolactone with or without furosemide, are first-line treatments. Patients with tense or refractory ascites may require therapeutic large-volume paracentesis, generally accompanied by albumin replacement when large volumes are removed. Transjugular intrahepatic portosystemic shunt (TIPS) may be considered for carefully selected patients with refractory ascites, while liver transplantation should be evaluated in appropriate patients with advanced cirrhosis.
