Cirrhosis: Understanding
Cirrhosis: Overview
Cirrhosis is a chronic, progressive liver disease characterized by extensive scarring (fibrosis) and the replacement of healthy liver tissue with scar tissue, which gradually impairs the liver's ability to function properly. The liver performs essential functions such as filtering toxins, producing proteins involved in blood clotting, storing nutrients, and supporting digestion. As cirrhosis progresses, increasing amounts of scar tissue disrupt the normal structure and blood flow within the liver, leading to reduced liver function and the development of serious complications. Although early-stage cirrhosis may not cause noticeable symptoms, advanced disease can result in liver failure and significantly impact overall health.
Cirrhosis develops as the result of long-term liver injury from a variety of underlying conditions. The most common causes include chronic viral hepatitis B and hepatitis C infections, long-term excessive alcohol consumption, and nonalcoholic fatty liver disease (NAFLD), particularly its progressive form known as nonalcoholic steatohepatitis (NASH). Other causes include autoimmune hepatitis, inherited metabolic disorders such as hemochromatosis and Wilson disease, diseases affecting the bile ducts including primary biliary cholangitis and primary sclerosing cholangitis, as well as prolonged exposure to certain medications, toxins, or chronic liver infections. Regardless of the underlying cause, persistent liver injury triggers inflammation and progressive scarring that eventually culminates in cirrhosis.
Cirrhosis develops when chronic liver injury causes persistent inflammation and activation of hepatic stellate cells, resulting in excessive collagen deposition and fibrosis. Progressive scar tissue and regenerative nodules disrupt the normal liver structure and blood flow, leading to portal hypertension and impaired liver function. Over time, these changes reduce the liver's ability to perform essential metabolic, synthetic, and detoxification functions, ultimately leading to liver failure if left untreated.
Cirrhosis is diagnosed using a combination of medical history, physical examination, blood tests, and imaging studies. Liver function tests help assess liver damage, while ultrasound, transient elastography (FibroScan), CT, or MRI evaluate liver structure and the extent of fibrosis. In some cases, a liver biopsy may be performed to confirm the diagnosis and determine disease severity.
Treatment of cirrhosis focuses on managing the underlying cause, slowing disease progression, and preventing complications. Depending on the cause, management may include antiviral therapy, alcohol cessation, weight reduction, or immunosuppressive treatment. Patients are also treated for complications such as portal hypertension, ascites, and hepatic encephalopathy, while liver transplantation remains the definitive treatment for advanced or decompensated cirrhosis.

It also helps to understand clinical trial details, expressive pharmacological action, agreements and collaborations, and the latest news and press releases.
Cirrhosis Emerging Drugs
 Efimosfermin Alfa: GSK
Efimosfermin Alfa (GSK6519754) is an investigational, once-monthly subcutaneous injection of a long-acting variant of fibroblast growth factor 21 (FGF21) being developed by GSK for metabolic dysfunction-associated steatohepatitis (MASH), including MASH with cirrhosis. The therapy is designed to regulate key metabolic pathways, thereby decreasing liver fat, ameliorating liver inflammation, and reversing liver fibrosis. Currently, the drug is being evaluated in the Phase III stage of its development for the treatment of Liver Cirrhosis.
 AZD2389: AstraZeneca
AZD2389 is an investigational, first-in-class, orally administered small-molecule fibroblast activation protein (FAP) inhibitor which selectively inhibits the enzymatic activity of FAP, a serine protease that is highly expressed in activated hepatic fibroblasts and is associated with liver fibrogenesis. AZD2389 is designed to inhibit FAP-mediated cleavage of substrates such as fibroblast growth factor 21 (FGF21) and ?2-antiplasmin (?2-AP), thereby preserving their activity and modulating pathways involved in liver fibrosis. Through inhibition of FAP, AZD2389 is intended to target the underlying fibrotic process and has the potential to slow fibrosis progression in patients with cirrhosis. Currently, the drug is being evaluated in the Phase II stage of its development for the treatment of Liver Cirrhosis.
 BI 3802876: Boehringer Ingelheim
BI 3802876 is an investigational therapeutic candidate being developed for the treatment of compensated cirrhosis due to metabolic dysfunction-associated steatohepatitis (MASH). The drug is administered as an intravenous infusion and is currently being evaluated in a Phase IIa clinical trial to assess its safety, tolerability, pharmacokinetics, and pharmacodynamics in patients with compensated cirrhosis. BI 3802876 is being developed with the aim of improving liver health in patients with this condition. However, the company has not publicly disclosed the drug's molecular target, mechanism of action, or the biological pathways through which it exerts its therapeutic effects. Currently, the drug is being evaluated in the Phase II stage of its development for the treatment of Cirrhosis.
 RTX001: Resolution Therapeutics
RTX001 is an engineered autologous Regenerative Macrophage Therapy (RMT) being developed by Resolution Therapeutics for the treatment of end-stage liver disease associated with decompensated liver cirrhosis. The therapy uses patient-derived macrophages engineered with IL-10 and MMP-9 mRNA to enhance their natural regenerative properties and increase their anti-inflammatory and anti-fibrotic activity, with the aim of promoting liver repair and improving the durability of therapeutic effects. Currently, the drug is being evaluated in the Phase I/II stage of its development for the treatment of Cirrhosis.
