Epstein–Barr virus (EBV) infection is caused by the Epstein–Barr virus, also known as human herpesvirus 4 (HHV-4), a ubiquitous double-stranded DNA virus belonging to the Herpesviridae family. EBV is one of the most prevalent human viruses, infecting more than 90% of the global adult population. The virus is primarily transmitted through saliva, although transmission can also occur via blood transfusions, organ transplantation, and, less commonly, sexual contact. Following primary infection, EBV establishes lifelong latency in B lymphocytes, with intermittent reactivation that enables viral shedding and transmission.
Primary EBV infection is often asymptomatic during childhood but commonly manifests as infectious mononucleosis in adolescents and young adults. Typical clinical features include fever, sore throat, cervical lymphadenopathy, fatigue, hepatosplenomegaly, and transient elevations in liver enzymes. While most cases are self-limiting and managed with supportive care, immunocompromised individuals are at an increased risk of severe EBV-related complications, including post-transplant lymphoproliferative disorder (PTLD), chronic active EBV infection, and other lymphoid proliferative diseases.
EBV is also recognized for its oncogenic potential and its association with several malignancies and immune-mediated disorders. Persistent latent infection has been linked to nasopharyngeal carcinoma, endemic Burkitt lymphoma, Hodgkin lymphoma, a subset of gastric carcinomas, and certain T-cell and NK-cell lymphomas. In addition, growing evidence suggests a role for EBV in the development of autoimmune diseases such as multiple sclerosis and systemic lupus erythematosus. Ongoing research is focused on advancing antiviral therapies, immunotherapies, adoptive T-cell therapies, and prophylactic and therapeutic vaccine candidates to improve the prevention and management of EBV-associated diseases.
