Essential Tremor: Understanding

Essential Tremor: Overview

Essential tremor (ET) is a common, chronic and progressive neurological movement disorder characterized primarily by involuntary, rhythmic action tremor, particularly affecting the hands and arms. It may also involve the head, voice, face, jaw and, less commonly, the legs or trunk. ET can occur at any age but becomes substantially more prevalent with advancing age, with population studies estimating prevalence around 1% overall and higher rates in older adults. The disorder can progressively interfere with activities such as writing, eating, drinking, dressing and other fine motor tasks. Increasing evidence also recognizes ET as a broader neurological syndrome that may include gait, cognitive, mood and speech abnormalities.

The characteristic manifestation is a bilateral postural and/or kinetic tremor, most prominently in the upper limbs, which becomes more evident when maintaining a posture or performing voluntary movements. Patients may experience difficulty writing, using utensils, holding cups, typing or performing other precise activities, while head and voice tremor can impair communication and social functioning. Tremor amplitude generally increases gradually over time, although the rate of progression varies considerably between individuals. Some patients experience mild gait or balance abnormalities, while anxiety, depression and subtle cognitive changes have also been reported. Alcohol may temporarily reduce tremor in some patients, although this is not considered a therapeutic strategy.

The precise cause of ET remains incompletely understood, and the disorder is considered biologically heterogeneous. A substantial proportion of patients have a positive family history, with many familial cases suggesting an autosomal-dominant pattern with variable genetic penetrance, while other cases occur sporadically. Current evidence implicates abnormal activity within the cerebello-thalamo-cortical motor network, involving the cerebellum, thalamus, motor cortex and related brainstem/olivary structures. Neuropathological studies have identified changes involving cerebellar Purkinje cells and surrounding neuronal networks in some patients, while abnormalities in GABAergic signaling and neuronal oscillations have also been proposed. These mechanisms may produce abnormal rhythmic motor activity that manifests clinically as tremor.

Diagnosis is primarily clinical, based on a detailed history and neurological examination demonstrating characteristic bilateral upper-limb action tremor while excluding Parkinson disease, dystonia, cerebellar disorders, medication-induced tremor, enhanced physiological tremor and functional tremor. The current MDS framework defines ET as an isolated tremor syndrome with bilateral upper-limb action tremor persisting for at least 3 years, without other clearly explanatory neurological signs. There is no single laboratory or imaging test that confirms ET, although investigations may be used to exclude alternative causes. Treatment is symptomatic and individualized; propranolol and primidone remain established first-line pharmacological options, while topiramate and other agents may be considered in selected patients. For medication-refractory disabling tremor, options include botulinum toxin, deep-brain stimulation, MRI-guided focused ultrasound thalamotomy and other neuromodulation approaches.

Essential Tremor Emerging Drugs

Ulixacaltamide: Praxis Precision Medicines

Ulixacaltamide (PRAX-944) is an investigational, highly selective small-molecule T-type calcium channel inhibitor developed by Praxis Precision Medicines for essential tremor (ET). It is designed to inhibit abnormal neuronal burst firing within the cerebello-thalamo-cortical circuit implicated in tremor generation. The drug demonstrated positive efficacy results in the Phase III Essential3 program and was generally well tolerated. In April 2026, the FDA accepted Praxis' NDA for ulixacaltamide in adults with ET, with a PDUFA target action date of January 29, 2027.
