Fuchs Endothelial Corneal Dystrophy (FECD) is a bilateral, slowly progressive corneal disorder characterized by the gradual loss and dysfunction of corneal endothelial cells and the formation of guttata, which are excrescences of the Descemet membrane. As endothelial cells deteriorate, the cornea loses its ability to regulate fluid balance, leading to corneal edema, visual impairment, glare, halos, and, in advanced stages, painful bullous keratopathy. FECD is the most common corneal endothelial dystrophy and a leading indication for corneal transplantation worldwide, primarily affecting individuals over 50 years of age and occurring more frequently in women.

The causes of Fuchs Endothelial Corneal Dystrophy (FECD) are multifactorial and involve a combination of genetic, cellular, and environmental factors. The disease is commonly inherited in an autosomal dominant pattern, although many cases occur sporadically. Genetic abnormalities, particularly expansions in the TCF4 gene, are strongly associated with late-onset FECD, while mutations in COL8A2, SLC4A11, ZEB1, AGBL1, and other genes have also been implicated. These genetic alterations contribute to progressive loss of corneal endothelial cells, impaired fluid regulation, and corneal edema. In addition, oxidative stress, mitochondrial dysfunction, hormonal influences, ultraviolet light exposure, smoking, and systemic conditions such as diabetes and hypertension may further promote endothelial cell damage and disease progression.

As endothelial function declines, the cornea loses its ability to regulate fluid balance, leading to corneal edema, reduced transparency, and impaired vision.
