Heart Failure With Reduced Ejection Fraction (HFrEF): Understanding

Heart Failure With Reduced Ejection Fraction (HFrEF): Overview

Heart failure with reduced ejection fraction (HFrEF) is a clinical syndrome characterized by the inability of the heart to pump sufficient blood to meet the body's metabolic demands due to impaired left ventricular systolic function, typically defined as a left ventricular ejection fraction (LVEF) of ?40%. It commonly results from ischemic heart disease, myocardial infarction, hypertension, dilated cardiomyopathy, valvular heart disease, or genetic and inflammatory disorders. Progressive ventricular remodeling, neurohormonal activation, and myocardial injury lead to declining cardiac output and worsening heart failure.

Patients typically present with exertional dyspnea, fatigue, reduced exercise tolerance, orthopnea, paroxysmal nocturnal dyspnea, peripheral edema, and weight gain due to fluid retention. HFrEF is associated with frequent hospitalizations, reduced quality of life, and substantial morbidity and mortality despite advances in therapy.

The pathophysiology of HFrEF involves loss of myocardial contractility accompanied by chronic activation of the sympathetic nervous system and the renin–angiotensin–aldosterone system (RAAS), which initially compensates for reduced cardiac output but ultimately promotes adverse ventricular remodeling, fibrosis, inflammation, and progressive systolic dysfunction. Increased ventricular wall stress, elevated filling pressures, endothelial dysfunction, and impaired renal perfusion contribute to fluid retention and worsening congestion.

Diagnosis is based on compatible clinical symptoms and signs together with objective evidence of reduced LVEF on transthoracic echocardiography, the cornerstone imaging modality. Additional evaluation includes measurement of B-type natriuretic peptide (BNP) or NT-proBNP, electrocardiography, chest radiography, laboratory investigations, cardiac magnetic resonance imaging when indicated, coronary assessment, and functional testing to identify the underlying cause and disease severity.

Treatment of HFrEF focuses on improving symptoms, slowing disease progression, reducing hospitalization, and lowering cardiovascular mortality through guideline-directed medical therapy (GDMT). The four foundational drug classes include an angiotensin receptor-neprilysin inhibitor (ARNI) or an ACE inhibitor/ARB when ARNI is not feasible, an evidence-based ?-blocker, a mineralocorticoid receptor antagonist (MRA), and a sodium-glucose cotransporter-2 (SGLT2) inhibitor. Loop diuretics are used to relieve congestion, while additional therapies such as ivabradine, vericiguat, hydralazine–isosorbide dinitrate, intravenous iron, and cardiac myosin activators may be appropriate for selected patients. Device therapies including implantable cardioverter-defibrillators (ICDs) and cardiac resynchronization therapy (CRT) are recommended for eligible patients, whereas left ventricular assist devices (LVADs) or heart transplantation are reserved for advanced refractory heart failure.

Heart Failure With Reduced Ejection Fraction (HFrEF) Emerging Drugs Chapters

This segment of the Heart Failure With Reduced Ejection Fraction (HFrEF) report encloses its detailed analysis of various drugs in different stages of clinical development, including Phase III, II, I, Preclinical and Discovery. It also helps to understand clinical trial details, expressive pharmacological action, agreements and collaborations, and the latest news and press releases.

Heart Failure With Reduced Ejection Fraction (HFrEF) Emerging Drugs

AC01: AnaCardio AB

AC01 is an investigational oral ghrelin receptor agonist with calcium-sensitizing inotropic properties being developed by AnaCardio for the treatment of chronic heart failure with reduced ejection fraction (HFrEF). The therapy is designed to enhance myocardial contractility without substantially increasing heart rate or myocardial oxygen demand, with the goal of improving cardiac function while maintaining a favorable safety profile. Early clinical studies have shown that AC01 is generally well tolerated and supports further evaluation in HFrEF patients. It is currently being investigated in the Phase II GOAL-HF2 trial to evaluate its safety and efficacy over 12 weeks in patients with chronic advanced HFrEF.

HJB647: Novartis

HJB647 is an investigational oral small-molecule therapy being developed by Novartis for the treatment of chronic heart failure with reduced ejection fraction (HFrEF) and mildly reduced ejection fraction (HFmrEF). Although its precise mechanism of action has not yet been publicly disclosed, the candidate is intended to improve cardiac function while maintaining an acceptable safety and tolerability profile. HJB647 is currently being evaluated in a randomized, double-blind, placebo-controlled Phase I crossover study assessing its safety, tolerability, and pharmacokinetics at two dose levels in patients with stable HFrEF/HFmrEF. The study will also characterize its pharmacokinetic profile to support further clinical development in heart failure.
