Neurotrophic Keratopathy (NK) is a rare, progressive degenerative disease of the cornea caused by impairment of trigeminal nerve innervation, leading to reduced or absent corneal sensation and defective epithelial healing. Loss of corneal nerve function can result in persistent epithelial defects, corneal ulceration, stromal melting, and, in severe cases, perforation and vision loss.
Common causes of NK include herpetic infections, diabetes, ocular surgery, trauma, and neurological disorders, all of which can damage the trigeminal nerve and impair corneal innervation. Despite progressive corneal damage, patients often experience few symptoms because of reduced corneal sensitivity, leading to a mismatch between clinical signs and patient complaints. Consequently, the disease is frequently overlooked or misdiagnosed and is often identified only at advanced stages when significant tissue damage has already occurred.
Diagnosis of NK is based on characteristic clinical findings and evaluation of corneal sensory function. Patients may present with punctate keratopathy, persistent epithelial defects, stromal ulceration, or corneal thinning that are disproportionate to symptoms due to impaired corneal sensitivity. Corneal sensation is assessed using instruments such as the Cochet–Bonnet esthesiometer or non-contact esthesiometry. Additional examinations, including slit-lamp biomicroscopy, fluorescein staining, and anterior segment imaging, help determine disease severity and stage and guide appropriate management.
Treatment of NK aims to promote corneal healing, prevent disease progression, and restore corneal innervation. Management is stage-dependent and includes preservative-free lubricants, therapeutic contact lenses, autologous serum eye drops, and amniotic membrane transplantation for persistent epithelial defects.
Cenegermin, a recombinant human nerve growth factor, is the first approved disease-modifying therapy and has demonstrated efficacy in stimulating corneal nerve regeneration and epithelial repair. In advanced cases with stromal ulceration or perforation, surgical interventions such as tarsorrhaphy, conjunctival flaps, corneal transplantation, or corneal neurotization may be required, while ongoing research is focused on regenerative therapies and novel neurotrophic agents.
