NVL-655 Market Summary

Key Factors Driving NVL-655 Growth

1. Strong activity in heavily pretreated ALK-positive NSCLC: The most important growth driver is the positive Phase I/II ALKOVE-1 data. Among 253 TKI-pretreated patients, including patients who had received a median of three prior lines of therapy, neladalkib achieved a 31% objective response rate (ORR) by blinded independent central review. Importantly, 64% of responders remained in response at 12 months and 53% at 18 months. In the particularly difficult lorlatinib-naïve subgroup, ORR reached 46%, with 80% and 60% of responses maintained at 12 and 18 months, respectively.

2. Potential to address ALK resistance mutations that limit existing TKIs: Resistance mutations are a major unmet need in ALK-positive NSCLC. In ALKOVE-1, 19% of the pivotal population had a secondary ALK G1202R mutation and 17% had compound ALK resistance mutations. Neladalkib produced a particularly strong 68% ORR in patients with G1202R, increasing to 83% among lorlatinib-naïve patients with G1202R. Preclinical work also showed ≥100-fold improved potency against ALK G1202R single and compound mutations compared with approved ALK TKIs.

3. Meaningful CNS activity could provide a major competitive advantage: Brain metastases are common in ALK-positive NSCLC, and approximately 40% of patients in the pivotal pretreated ALKOVE-1 population had active CNS disease at baseline. Among 92 patients with measurable CNS lesions, neladalkib achieved an intracranial ORR of 32%, while the lorlatinib-naïve subgroup achieved 63% intracranial ORR. Importantly, intracranial responses were also observed in patients previously treated with lorlatinib. The drug's brain penetration and CNS activity could therefore support differentiation in a clinically important segment.

4. Potential expansion into the first-line ALK-positive NSCLC market: The commercial opportunity could expand substantially if neladalkib demonstrates superiority or non-inferiority to established first-line therapy. Preliminary data from 44 TKI-naïve patients showed an 86% ORR, with 91% of responders maintaining response for at least 12 months. Intracranial ORR was 78% (7/9), with no CNS progression among responders at the data cutoff. These results are being tested in the ongoing ALKAZAR Phase III trial, which directly compares neladalkib with alectinib in treatment-naïve ALK-positive advanced NSCLC.

5. Potentially differentiated tolerability through ALK selectivity/TRK sparing: Neladalkib was deliberately designed to inhibit ALK while minimizing off-target TRK inhibition, which Nuvalent believes may help address CNS and tolerability issues associated with some existing ALK TKIs. In the 656 patients treated at the recommended Phase II dose, only 5% discontinued treatment because of treatment-emergent adverse events, while 17% required dose reductions. The most common adverse events were mainly laboratory transaminase elevations, constipation, dysgeusia, peripheral edema, cough and nausea.

NVL-655 Recent Developments

In May 2026, the FDA accepted the neladalkib NDA for filing and granted Priority Review, with a November 27, 2026 PDUFA date. More importantly, GSK completed its acquisition of Nuvalent in July 2026 for approximately $10.6 billion, gaining neladalkib and zidesamtinib. GSK described neladalkib as a potential best-in-class asset with multi-blockbuster potential and expects both lead assets to contribute to revenue growth from 2027 if approved.

NVL-655 Drug Summary

NVL-655, now known as Neladalkib, is an investigational, next-generation, orally administered, brain-penetrant and highly selective anaplastic lymphoma kinase (ALK) tyrosine kinase inhibitor (TKI) developed by Nuvalent for the treatment of ALK-positive non-small cell lung cancer (NSCLC). The drug is rationally designed to inhibit ALK and a broad range of treatment-emergent resistance mutations, including single and compound mutations involving G1202R, while sparing the structurally related tropomyosin receptor kinase (TRK) family to potentially reduce TRK-related central nervous system (CNS) adverse effects. Neladalkib is being developed to address both systemic and CNS disease, including tumors that have progressed following first-, second-, and third-generation ALK inhibitors. In clinical development, the recommended Phase II dose was established at 150 mg once daily, and early ALKOVE-1 data demonstrated durable systemic and intracranial responses in heavily pretreated patients. As of 2026, the drug has received FDA Breakthrough Therapy and Orphan Drug designations for ALK-positive NSCLC, while its New Drug Application for previously treated advanced ALK-positive NSCLC has been accepted for Priority Review with a PDUFA target action date of November 2026; a Phase III ALKAZAR trial is also evaluating neladalkib against alectinib in treatment-naïve ALK-positive advanced NSCLC. The report provides NVL-655’s sales, growth barriers and drivers, post usage and approvals in multiple indications.
