Onychomycosis Understanding

Onychomycosis, a fungal infection of the nail, is the most common nail infection worldwide, causing discoloration and thickening of the affected nail plate. Onychomycosis was initially thought to be caused by dermatophytes predominantly; however, new research has revealed that mixed infections and those caused by nondermatophyte molds (NDMs) are more prevalent than thought previously, especially in warmer climates. 


In post‐industrialized countries, more than 10% of the general population is reported to have onychomycosis, according to the study by Thomas et al. The prevalence of Onychomycosis has been increasing in the United States over the years with various studies pointing towards it. A 1979 United States study (n=20000) reported the overall prevalence of onychomycosis as low as 2·18%. A United States study in 1997 (n=1038) revealed a much higher prevalence of onychomycosis (8·7%) and a large‐scale multicentre North American study published in 2000 (n=1832) estimated the prevalence of onychomycosis to be 13·8%.


The study published by Dyanne et al. states that Onychomycosis occurs in 10% of the general population but is more common in older adults; the prevalence is 20% in those older than 60 years and 50% in those older than 70 years. The increased prevalence in older adults is related to peripheral vascular disease, immunologic disorders, and diabetes mellitus. The risk of onychomycosis is 1.9 to 2.8 times higher in persons with diabetes compared with the general population. In patients with human immunodeficiency virus infection, the prevalence ranges from 15% to 40%.
