Peritoneal cancer refers to malignant tumors involving the peritoneum, the thin membrane lining the abdominal cavity and covering many abdominal organs. It may arise primarily from the peritoneum, such as primary peritoneal carcinoma or malignant peritoneal mesothelioma, or more commonly represent peritoneal metastases (peritoneal carcinomatosis) from ovarian, colorectal, gastric, appendiceal and other gastrointestinal cancers. Peritoneal involvement is considered an advanced pattern of cancer spread and can be associated with substantial morbidity.
Clinical manifestations vary with tumor burden and the underlying primary cancer, but commonly include abdominal distension, pain or discomfort, increasing abdominal girth, nausea, vomiting, reduced appetite and unintended weight loss. Ascites is particularly common and may cause early satiety, breathlessness and progressive abdominal swelling. Advanced disease can produce bowel or ureteral obstruction, altered bowel habits, fatigue and nutritional deterioration, while some patients remain relatively asymptomatic until the peritoneal disease becomes extensive.
Peritoneal malignancy most frequently develops through transcoelomic dissemination, in which tumor cells detach from a primary abdominal or pelvic tumor, enter the peritoneal cavity and implant on peritoneal surfaces. Tumor cells can subsequently proliferate, invade the peritoneal tissue and spread along pathways of peritoneal fluid circulation, producing multiple nodules and infiltrative deposits. Ovarian and gastrointestinal cancers have a particularly strong tendency toward peritoneal spread, while tumor biology, stage, histology and the extent of primary disease influence the risk and pattern of dissemination.
Diagnosis generally combines clinical assessment, laboratory investigations and imaging, particularly contrast-enhanced CT to identify ascites, peritoneal nodules, omental infiltration and other metastatic sites; diagnostic laparoscopy and biopsy may be required for definitive characterization and assessment of disease extent. Treatment depends on the primary tumor, distribution and volume of peritoneal disease, and patient fitness, and may include systemic chemotherapy, targeted or immunotherapies where appropriate, and palliative procedures. In carefully selected patients with limited peritoneal disease, cytoreductive surgery (CRS) with or without hyperthermic intraperitoneal chemotherapy (HIPEC) may provide prolonged disease control; however, patient selection and the feasibility of complete cytoreduction are critical.

Raludotatug deruxtecan (R-DXd) is an investigational CDH6-directed antibody–drug conjugate (ADC) developed by Daiichi Sankyo and jointly developed globally with Merck & Co. for CDH6-expressing solid tumors, including ovarian, primary peritoneal and fallopian tube cancers. It consists of a humanized anti-CDH6 antibody linked to a topoisomerase I inhibitor DXd payload, designed to selectively deliver cytotoxic therapy to CDH6-expressing tumor cells. For peritoneal cancer, R-DXd is being evaluated in the Phase III trial in patients with platinum-resistant, high-grade ovarian, primary peritoneal or fallopian tube cancer. The program has received FDA Breakthrough Therapy Designation for CDH6-expressing platinum-resistant ovarian, primary peritoneal or fallopian tube cancers previously treated with bevacizumab.
Utidelone: Beijing Biostar Pharmaceuticals
Utidelone (UTD2) is an investigational oral epothilone analog and next-generation microtubule stabilizer developed by Beijing Biostar Pharmaceuticals for advanced cancers, including primary peritoneal cancer. It binds to tubulin, promotes microtubule polymerization and prevents depolymerization, thereby disrupting mitosis and inducing apoptosis in tumor cells. For platinum-resistant advanced epithelial ovarian, fallopian tube, or primary peritoneal cancer, Utidelone capsules are being evaluated in Phase II study. The study is assessing different once-daily and twice-daily dosing regimens, with safety, dose-limiting toxicity, objective response rate, disease control rate and progression-free survival among the key outcomes.
TORL-1-23: TORL BioTherapeutics
TORL-1-23 (ixotatug vedotin) is an investigational Claudin-6 (CLDN6)-targeted antibody–drug conjugate (ADC) developed by TORL BioTherapeutics for CLDN6-positive solid tumors, including ovarian, fallopian tube and primary peritoneal cancers. The ADC is designed to selectively bind CLDN6-expressing tumor cells and deliver its cytotoxic payload, thereby inducing tumor-cell death while limiting exposure to normal tissues with little or no CLDN6 expression. For peritoneal cancer, it is being evaluated in the CATALINA-2 Phase II study in women with CLDN6-positive platinum-resistant ovarian, primary peritoneal or fallopian tube cancer who have received 1–3 prior lines of therapy. TORL-1-23 is currently in Phase II development, with objective response rate as the primary efficacy endpoint; the program has also received FDA Fast Track Designation.
Sovilnesib: Volastra Therapeutics
Sovilnesib (formerly AMG 650) is an investigational oral KIF18A inhibitor developed by Volastra Therapeutics for cancers characterized by high chromosomal instability, including ovarian, fallopian tube and primary peritoneal cancer. By inhibiting KIF18A, a mitotic kinesin, sovilnesib is designed to disrupt chromosome segregation and preferentially induce cell death in tumors with high chromosomal instability. For peritoneal cancer, it is being evaluated in Phase Ib study in patients with platinum-resistant or refractory high-grade serous ovarian, fallopian tube or primary peritoneal cancer. The randomized dose-optimization study evaluates once-daily oral sovilnesib across multiple dose levels, with the aim of establishing the recommended Phase II dose (RP2D) while assessing safety, pharmacokinetics and preliminary efficacy.
