Plaque Psoriasis: Understanding

Plaque Psoriasis: Overview

Plaque psoriasis, also known as psoriasis vulgaris, is a chronic, immune-mediated inflammatory skin disorder and the most common form of psoriasis, accounting for approximately 80%–90% of cases. It is characterized by well-demarcated, erythematous plaques covered with silvery-white scales, most commonly affecting the extensor surfaces of the elbows and knees, scalp, trunk, and lumbosacral region. The disease generally follows a lifelong relapsing-remitting course, with periods of exacerbation and remission. In addition to cutaneous manifestations, plaque psoriasis may involve the nails and joints and is associated with systemic comorbidities, including psoriatic arthritis, cardiovascular disease, metabolic syndrome, and depression.

The development of plaque psoriasis is multifactorial and involves an interaction between genetic predisposition, immune dysregulation, and environmental factors. Genetic susceptibility has an important role, with several psoriasis-associated loci identified, particularly PSORS1 on chromosome 6p21, which includes the HLA-Cw6 allele and is strongly associated with early-onset and more severe disease. In genetically predisposed individuals, various environmental and lifestyle factors can trigger or exacerbate disease. These include infections, particularly streptococcal infections, physical trauma to the skin (Koebner phenomenon), psychological stress, certain medications such as beta-blockers, lithium, antimalarials and immune checkpoint inhibitors, abrupt withdrawal of corticosteroids, smoking, alcohol use, obesity, and metabolic syndrome.

Plaque psoriasis results from dysregulated interactions between the innate and adaptive immune systems, leading to persistent inflammation and abnormal keratinocyte proliferation and differentiation. Antigen-presenting cells activate T cells, which migrate to the skin and release pro-inflammatory cytokines, particularly through the IL-23/IL-17 axis and TNF-? pathway. These signals recruit and activate additional immune cells, including dendritic cells, neutrophils, and macrophages, thereby amplifying cutaneous inflammation. Cytokine-mediated activation of keratinocytes causes rapid epidermal proliferation, acanthosis, parakeratosis, and impaired differentiation. Keratinocyte turnover is markedly accelerated compared with normal skin, resulting in epidermal thickening and the characteristic scaly plaques.

The diagnosis of plaque psoriasis is primarily clinical and is established through a detailed medical history and physical examination. Typical findings include symmetrically distributed, sharply demarcated erythematous plaques with dry, silvery-white scales, particularly on the elbows, knees, scalp, and lumbosacral region. Nail findings such as pitting, onycholysis, subungual hyperkeratosis, and oil-drop discoloration may support the diagnosis and can also indicate an increased likelihood of psoriatic arthritis. A skin biopsy is generally not required for classic presentations but may be performed when lesions are atypical or when other disorders need to be excluded. Disease severity is commonly assessed using measures such as the Psoriasis Area and Severity Index (PASI), Body Surface Area (BSA), and Dermatology Life Quality Index (DLQI).

Treatment of plaque psoriasis is individualized according to disease severity, affected body surface area, quality-of-life impact, comorbidities, and response to previous therapy. Topical therapies are generally used as first-line treatment for mild disease and include corticosteroids, vitamin D analogs, retinoids, coal tar, anthralin, and salicylic acid. Phototherapy, particularly narrowband UVB, may be used when topical therapy is inadequate or disease is more widespread. Patients with moderate-to-severe disease may require systemic treatments such as methotrexate, cyclosporine, acitretin, or targeted oral therapies including apremilast and selected JAK inhibitors. Biologic therapies targeting TNF-?, IL-17, IL-12/23, or IL-23 are important treatment options for moderate-to-severe disease and can provide substantial and durable skin clearance.

It also helps to understand clinical trial details, expressive pharmacological action, agreements and collaborations, and the latest news and press releases.

Plaque Psoriasis Emerging Drugs

  • Piclidenoson: Can-Fite BioPharma

Piclidenoson is an orally bioavailable, highly selective A3 adenosine receptor agonist with anti-inflammatory activity demonstrated in both preclinical and clinical studies. The drug has accumulated an extensive clinical safety database involving more than 1,500 subjects and is being developed as a potential chronic therapy for psoriasis and other inflammatory diseases. The drug’s mechanism of action entails inhibition of the inflammatory cytokines interleukin 17 and 23 (IL-17 and IL-23) and the induction of apoptosis of patients’ skin cell keratinocytes involved with the disease pathogenicity. Currently, the drug is being evaluated in the Phase III stage of its development for the treatment of Plaque Psoriasis.

  • ORKA-001: Oruka Therapeutics

ORKA-001 is an investigational, half-life-extended monoclonal antibody being developed for the treatment of moderate-to-severe plaque psoriasis. The drug selectively targets the p19 subunit of interleukin-23 (IL-23p19), a cytokine that plays a key role in the inflammatory pathway underlying psoriasis. By binding to IL-23p19, ORKA-001 is designed to block IL-23 signaling and thereby inhibit downstream inflammatory activity associated with psoriatic disease. Oruka has engineered ORKA-001 with an extended half-life with the aim of maintaining durable IL-23 pathway suppression and enabling substantially less frequent dosing than conventional biologic therapies. Currently, the drug is being evaluated in the Phase II stage of its development for the treatment of Plaque Psoriasis.

  • AX-158: Artax Biopharma

AX-158 is an investigational, orally administered small-molecule Nck modulator which selectively modulates T cell receptor (TCR) signaling by binding to the SH3.1 domain of Nck and inhibiting the interaction between Nck and the TCR. Nck normally binds to the TCR following antigen recognition and amplifies T cell responses, particularly to low-affinity antigens that can contribute to autoimmune pathology. By modulating Nck-mediated TCR signaling, AX-158 is designed to selectively reduce the amplification of autoreactive T cell responses while preserving normal immune responses to foreign antigens. Currently, the drug is being evaluated in the Phase II stage of its development for the treatment of Plaque Psoriasis.

  • ASC50: Ascletis Pharma

ASC50 is an investigational, orally administered small-molecule interleukin-17 (IL-17) inhibitor which is designed to selectively target and inhibit the IL-17 pathway, a key inflammatory pathway involved in the pathogenesis of psoriasis. By inhibiting IL-17 signaling, ASC50 is intended to reduce IL-17-driven inflammatory responses and thereby alleviate the skin inflammation and pathological changes associated with psoriasis. Ascletis has reported that ASC50 demonstrated pharmacological activity in a psoriasis animal model, where oral administration reduced PASI score, skin thickness, inflammation score, and dermal thickness, with effects comparable to an IL-17 monoclonal antibody in the model. Currently, the drug is being evaluated in the Phase I stage of its development for the treatment of Plaque Psoriasis.

  • ZP9830: Zealand Pharma

ZP9830 is an investigational, potent and selective Kv1.3 ion channel blocker being developed by Zealand Pharma for the treatment of immune-mediated inflammatory diseases, including plaque psoriasis. ZP9830 targets Kv1.3, a voltage-gated potassium channel that is highly expressed on effector memory T cells and plays an important role in the activation, proliferation, migration, and cytokine production of leukocytes. By selectively inhibiting Kv1.3, ZP9830 is designed to suppress the activity and inflammatory responses of effector memory T cells and other immune cells involved in chronic inflammation, while potentially preserving the functions of other components of the immune system. Currently, the drug is being evaluated in the Phase I stage of its development for the treatment of Plaque Psoriasis.
