Polycystic Ovary Syndrome (PCOS), officially renamed as PMOS in 2026, is one of the most common endocrine and metabolic disorders affecting women of reproductive age. The new terminology more accurately reflects the multisystem nature of the disease, recognizing that it extends beyond the ovaries and that many affected individuals do not have polycystic ovaries. PMOS is characterized by a complex interaction of hormonal imbalance, insulin resistance, chronic low-grade inflammation, and ovarian dysfunction, leading to hyperandrogenism, ovulatory dysfunction, and variable ovarian morphology. In addition to reproductive complications such as infertility and menstrual irregularities, the disorder is associated with obesity, T2D, dyslipidemia, hypertension, metabolic dysfunction-associated steatotic liver disease (MASLD), and psychological disorders, including anxiety and depression. Owing to its lifelong clinical burden, high prevalence, and broad metabolic implications, PMOS remains a significant public health challenge with substantial unmet medical needs, particularly given the absence of disease-specific approved therapies.

Polyendocrine Metabolic Ovarian Syndrome (PMOS) Diagnosis

The diagnosis of PMOS, is based on a combination of clinical, biochemical, and imaging findings, as no single diagnostic test exists. The Rotterdam criteria remain the most widely accepted diagnostic standard, requiring the presence of at least two of the following after excluding other endocrine disorders: oligo- or anovulation, clinical or biochemical hyperandrogenism, and polycystic ovarian morphology on ultrasound. A comprehensive evaluation includes assessment of serum androgen levels, thyroid function, prolactin, and adrenal hormones to rule out alternative causes of menstrual irregularities and androgen excess. Given the syndrome's strong metabolic component, patients are also routinely screened for obesity, insulin resistance, impaired glucose tolerance or T2D, dyslipidemia, hypertension, and other cardiometabolic risk factors to guide long-term management.
