Psoriatic Arthritis: Understanding

Psoriatic Arthritis: Overview

Psoriatic arthritis (PsA) is a chronic, heterogeneous inflammatory arthritis associated with psoriasis that can affect the peripheral joints, axial skeleton, entheses, digits, skin, nails, and other organs. It is generally a seronegative inflammatory arthritis, although a small proportion of patients may have rheumatoid factor or anti-cyclic citrullinated peptide antibodies. The clinical presentation is variable, with patterns including asymmetric oligoarthritis, symmetric polyarthritis, distal interphalangeal joint arthritis, arthritis mutilans, and axial disease. Psoriasis usually precedes arthritis, but joint manifestations may occur simultaneously with or before the onset of skin disease, which can complicate diagnosis.

The exact etiology of PsA is not fully understood and is thought to result from an interaction between genetic susceptibility and environmental factors that leads to immune-mediated inflammation. Genetic factors include associations with several HLA and non-HLA genes involved in antigen presentation, immune recognition, cytokine signaling, and T-cell function. Approximately one-third to one-half of patients have a first-degree relative with psoriasis or PsA. Environmental factors that may contribute to disease development or flares include infections, antibiotic exposure, and mechanical trauma to the skin, joints, or entheses.

PsA develops through an immune-mediated inflammatory process in genetically susceptible individuals, often following environmental triggers such as infection or mechanical stress. The IL-23/IL-17 axis plays a central role in disease pathogenesis. IL-23 produced by macrophages and dendritic cells stimulates immune cells, including T-cell populations, leading to production of pro-inflammatory cytokines such as IL-17, IL-22, and TNF-?. These mediators promote recruitment of inflammatory cells, activation of synoviocytes and osteoclasts, angiogenesis, bone erosion, and abnormal new bone formation. Enthesitis, or inflammation at tendon, ligament, and joint-capsule attachment sites, is considered a characteristic pathological feature of PsA and contributes to manifestations such as dactylitis and distal interphalangeal joint involvement.

There is no single laboratory test that definitively diagnoses PsA, so diagnosis relies on clinical assessment, physical examination, laboratory investigations, imaging, and classification criteria. Patients are evaluated for inflammatory arthritis as well as characteristic features such as psoriasis, nail changes, enthesitis, dactylitis, and axial involvement. ESR and CRP may be elevated but can be normal and therefore cannot exclude PsA. Rheumatoid factor and anti-CCP antibodies are generally negative, although they can occasionally be positive.

Treatment of PsA aims to control inflammation and symptoms, prevent structural joint damage, preserve physical function, and improve quality of life. Management is individualized according to the affected disease domains, disease severity, joint damage, skin and nail involvement, comorbidities, and patient preferences. Non-pharmacological approaches include exercise, physical and occupational therapy, and smoking cessation. NSAIDs may be used for symptomatic relief in mild disease, while conventional synthetic DMARDs such as methotrexate or sulfasalazine may be used for peripheral arthritis. Patients with more severe or persistent disease may require biologic DMARDs, including TNF, IL-17, or IL-12/23 pathway inhibitors, while targeted synthetic therapies such as JAK inhibitors and PDE-4 inhibitors may also be considered in appropriate patients.

Psoriatic Arthritis Emerging Drugs Chapters

This segment of the Psoriatic Arthritis report encloses its detailed analysis of various drugs in different stages of clinical development, including phase II, I, preclinical and Discovery. It also helps to understand clinical trial details, expressive pharmacological action, agreements and collaborations, and the latest news and press releases.

Psoriatic Arthritis Emerging Drugs

  • Zasocitinib: Takeda

Zasocitinib (TAK-279) is an investigational, next-generation, highly selective oral tyrosine kinase 2 (TYK2) inhibitor being developed by Takeda for psoriatic arthritis (PsA). Takeda describes zasocitinib as a potent allosteric TYK2 inhibitor designed to maintain sustained inhibition of IL-23 and other core disease-driving immune pathways involved in immune-mediated inflammatory diseases. The molecule has more than 1-million-fold greater selectivity for TYK2 than other JAK enzymes, with the potential to inhibit TYK2 while avoiding effects on JAK1, JAK2, and JAK3 signaling based on in vitro data. Currently, the drug is being evaluated in the Phase III stage of its development for the treatment of Psoriatic Arthritis.

  • Tulisokibart: Merck & Co., Inc.

Tulisokibart (MK-7240) is an investigational humanized monoclonal antibody which is directed against tumor necrosis factor-like ligand 1A (TL1A), a cytokine associated with inflammation and fibrosis. By targeting TL1A, tulisokibart is designed to modulate this inflammatory pathway and potentially reduce disease activity. Currently, the drug is being evaluated in the Phase II stage of its development for the treatment of Psoriatic Arthritis.

  • SPY072: Spyre Therapeutics, Inc.

SPY072 is an investigational, extended half-life monoclonal antibody being developed by Spyre Therapeutics for rheumatic diseases, including psoriatic arthritis (PsA). Spyre describes SPY072 as an anti-TL1A antibody targeting tumor necrosis factor-like ligand 1A (TL1A), a cytokine that the company describes as central to T-cell-driven inflammation. By inhibiting TL1A, SPY072 is designed to block this inflammatory pathway and potentially reduce inflammation associated with rheumatic diseases such as PsA. Spyre has engineered SPY072 for enhanced potency, durability, and pharmacokinetics, with the potential for quarterly or twice-yearly subcutaneous maintenance dosing. Currently, the drug is being evaluated in the Phase II stage of its development for the treatment of Psoriatic Arthritis.

  • si-544: selectION Therapeutics

si-544 is the lead drug candidate of selectION Therapeutics and is an investigational immuno-selective agent targeting Kv1.3, a specific ion channel involved in the activation and proliferation of effector memory T (TEM) cells. The company describes si-544 as a potent Kv1.3 blocker with what it believes to be class-leading selectivity. TEM cells are implicated in several autoimmune diseases, including psoriasis and rheumatoid arthritis, which provides the rationale for investigating si-544 in immune-mediated diseases such as psoriatic arthritis. By blocking Kv1.3, si-544 is designed to functionally inhibit and eliminate disease-specific, chronically activated TEM cells while maintaining full immunocompetence. Currently, the drug is being evaluated in the Phase I stage of its development for the treatment of Psoriatic Arthritis.
