Sinusitis, more accurately termed rhinosinusitis, is an inflammatory disorder involving the mucosa of the nasal cavity and one or more paranasal sinuses. It may be classified according to duration as acute, subacute, or chronic, with chronic rhinosinusitis (CRS) generally defined by symptoms persisting for 12 weeks or longer. CRS is further divided into chronic rhinosinusitis with nasal polyps (CRSwNP) and without nasal polyps (CRSsNP). Unlike acute disease, CRS is primarily regarded as a chronic inflammatory condition rather than simply an infection.

Common signs and symptoms include nasal congestion or obstruction, nasal discharge or postnasal drip, facial pain or pressure, and reduced or lost sense of smell. Patients may also experience headache, facial tenderness, cough, fatigue, halitosis, tooth discomfort and, particularly in acute disease, fever or malaise. Symptoms can vary according to disease phenotype and severity, with nasal polyps, asthma and allergic disease frequently occurring as associated conditions. Persistent symptoms can substantially affect sleep, daily functioning and health-related quality of life.

The causes and underlying mechanisms are multifactorial, involving infection, mucosal inflammation, impaired sinus drainage, allergy, anatomical abnormalities and environmental factors. Treatment depends on disease type and severity and generally begins with conservative measures such as saline nasal irrigation and topical intranasal corticosteroids; antibiotics may be used when bacterial infection is suspected rather than routinely for inflammatory CRS. Persistent or severe disease may require systemic corticosteroids in selected cases, endoscopic sinus surgery, and for appropriately selected severe CRSwNP patients, targeted biologic therapies.

HTMC0658: Shanghai Huilun Pharmaceutical

HTMC0658 (also known as YD-0293) is an investigational small-molecule DPP-1 (dipeptidyl peptidase-1/cathepsin C) inhibitor being developed by Shanghai Yidian Pharmaceutical Technology Development Co., Ltd., a subsidiary of Shanghai Huilun Pharmaceutical for chronic rhinosinusitis without nasal polyps (CRSsNP). HTMC0658 is currently being evaluated in a Phase IIa clinical trial, in which it is administered orally once daily for 12 weeks alongside mometasone furoate nasal spray. The trial is assessing changes in sinus CT findings (Lund-MacKay score), symptoms and quality of life, including SNOT-22, nasal congestion and rhinosinusitis severity, supporting its development as a potential disease-modifying treatment for CRSsNP.

VVN432: VivaVision Biotech

VVN432 is an investigational nasal spray being developed by VivaVision Biotech, Inc. for chronic rhinosinusitis (CRS). The clinical program is evaluating 0.5% and 1% VVN432 nasal spray, administered intranasally, for safety, tolerability, pharmacokinetics/pharmacodynamics, and preliminary efficacy. The Phase Ib portion specifically enrolls patients with chronic rhinosinusitis, with or without nasal polyps, and assesses measures including nasal congestion and total symptom scores. VVN432 is currently in Phase I development.
