Squamous cell carcinoma of the lung (LUSC or lung SCC) is a major histological subtype of non-small cell lung cancer (NSCLC), accounting for approximately 20–30% of all lung cancers. It originates from the squamous epithelial cells lining the central bronchi and is strongly associated with long-term tobacco smoking, although occupational exposures such as asbestos, radon, and heavy metals also contribute to disease risk. Lung SCC typically arises in the central airways and has distinct pathological and molecular characteristics compared with lung adenocarcinoma.
Patients with early-stage lung SCC are frequently asymptomatic, resulting in delayed diagnosis. As the tumor enlarges, common manifestations include persistent cough, hemoptysis, dyspnea, chest pain, wheezing, hoarseness, recurrent respiratory infections, fatigue, anorexia, and unexplained weight loss. Advanced disease may present with symptoms related to metastatic spread, including bone pain, neurological deficits, headaches, seizures, or spinal cord compression. The clinical presentation largely depends on tumor size, airway obstruction, and metastatic involvement.
The pathogenesis of lung SCC begins with chronic exposure to tobacco carcinogens, leading to squamous metaplasia, epithelial dysplasia, carcinoma in situ, and ultimately invasive carcinoma. Molecular alterations commonly involve TP53 mutations, CDKN2A inactivation, SOX2 amplification, FGFR1 amplification, PI3K pathway abnormalities, and KEAP1/NFE2L2 mutations, which promote uncontrolled cellular proliferation, impaired apoptosis, genomic instability, and resistance to oxidative stress. Histologically, tumors are characterized by keratinization and intercellular bridges, with immune-histochemical markers such as p40 and p63 supporting diagnosis.
Diagnosis is established through a combination of clinical evaluation, contrast-enhanced chest CT, PET-CT for staging, bronchoscopy with tissue biopsy for centrally located tumors, and histopathological examination with immunohistochemistry. Molecular profiling and PD-L1 testing are increasingly performed to guide therapeutic decisions.
