T-cell Acute Lymphoblastic Leukemia (T-ALL) is an aggressive hematologic malignancy arising from the malignant transformation of immature T-cell precursors. It accounts for approximately 12–15% of pediatric and 20–25% of adult acute lymphoblastic leukemia (ALL) cases and is characterized by rapid proliferation of leukemic T cells in the bone marrow, blood, and other tissues. Despite its aggressive nature, advances in intensive chemotherapy and risk-adapted treatment strategies have significantly improved survival outcomes, although relapse remains associated with a poor prognosis. 

T-cell Acute Lymphoblastic Leukemia (T-ALL) is primarily caused by the accumulation of genetic alterations in immature T-cell precursors that disrupt normal cell growth, differentiation, and survival. Key drivers include activating mutations in the NOTCH1 signaling pathway, loss of tumor suppressor genes such as CDKN2A/CDKN2B, and abnormal activation of transcription factors including TAL1, TLX1, and TLX3. These changes promote uncontrolled proliferation of leukemic T cells and contribute to disease development. 

T-cell Acute Lymphoblastic Leukemia (T-ALL) develops through the malignant transformation of immature T-cell precursors driven by the accumulation of genetic abnormalities that disrupt normal T-cell development. Aberrant activation of the NOTCH1 signaling pathway, along with alterations in transcription factors such as TAL1, TLX1, and TLX3 and loss of cell-cycle regulators like CDKN2A/CDKN2B, promotes uncontrolled proliferation, impaired differentiation, and prolonged survival of leukemic cells. These abnormal lymphoblasts accumulate in the bone marrow and peripheral blood, suppress normal hematopoiesis, and may infiltrate extramedullary tissues such as the lymph nodes, mediastinum, and central nervous system. 

T-cell Acute Lymphoblastic Leukemia (T-ALL) is diagnosed through morphological examination of blood and bone marrow samples, immunophenotyping to confirm T-cell lineage, and cytogenetic and molecular analyses to identify characteristic genetic abnormalities. Assessment of minimal residual disease (MRD) is also important for risk stratification and treatment monitoring. 

Treatment of T-ALL primarily involves intensive multi-agent chemotherapy administered in induction, consolidation, and maintenance phases. High-risk or relapsed patients may undergo allogeneic hematopoietic stem cell transplantation.
