Ulcerative Colitis: Overview
Ulcerative colitis (UC) is a chronic, idiopathic inflammatory bowel disease characterized by diffuse inflammation, superficial erosions, and ulceration of the colon. The inflammation is primarily limited to the mucosa and submucosa and typically begins in the rectum before extending proximally in a continuous pattern. UC follows a relapsing-remitting course and commonly presents with bloody diarrhea, abdominal pain, urgency, and tenesmus.
The exact cause of UC remains unknown, but it is thought to result from a combination of genetic susceptibility, abnormal immune responses, alterations in the gut microbiota, and defects in the intestinal mucosal barrier. A family history of UC is an important risk factor, while dysregulated mucosal immunity and an abnormal immune response to normally non-pathogenic intestinal bacteria are also considered contributors.
UC involves disruption of the colonic epithelial barrier, altered gut microbiota, and an inappropriate immune response. Defects in mucin and tight-junction function can increase exposure of the mucosa to luminal antigens, triggering activation of immune cells. This promotes inflammatory signaling involving mediators such as tumor necrosis factor-alpha (TNF-?) and interleukin-13, along with increased leukocyte recruitment into the colonic mucosa. The resulting persistent inflammation causes epithelial injury, crypt abnormalities, ulceration, and depletion of goblet cells.
Diagnosis is based on clinical presentation together with endoscopic and histological findings and exclusion of infectious causes. Stool testing is performed to rule out bacterial and parasitic infections. Colonoscopy or proctosigmoidoscopy typically demonstrates loss of the vascular pattern, mucosal granularity and friability, erosions, and ulceration in a continuous pattern beginning in the rectum. Multiple biopsies are obtained for histological confirmation. Laboratory markers such as C-reactive protein (CRP), erythrocyte sedimentation rate (ESR), and fecal calprotectin can support assessment of inflammation, although they are not specific for UC.
Treatment is determined by disease extent and severity, with the goals of inducing and maintaining remission and preventing complications. Mild-to-moderate disease is commonly treated with 5-aminosalicylates (5-ASAs), administered orally and/or rectally, while corticosteroids may be used to induce remission when 5-ASAs are insufficient. Patients with refractory disease may require immunosuppressants such as azathioprine or 6-mercaptopurine and biologic therapies, including anti-TNF agents such as infliximab, adalimumab, and golimumab, or the anti-integrin agent vedolizumab. Colectomy can provide a definitive cure for colonic disease and is considered for medically refractory or complicated UC, including fulminant colitis, toxic megacolon, perforation, severe bleeding, dysplasia, or colorectal cancer.
