The development of vulvar cancer is associated with multiple risk factors, including increasing age, high-risk human papillomavirus (HPV) infection, smoking, chronic inflammatory conditions of the vulva, previous pelvic radiation, and immunodeficiency. Vulvar SCC can arise through HPV-dependent and HPV-independent pathways, with chronic vulvar inflammatory conditions contributing particularly to HPV-independent disease.

Vulvar SCC develops primarily through two molecular pathways. In approximately 30%–40% of cases, high-risk HPV infection contributes to carcinogenesis through the viral E6 and E7 oncoproteins, which inactivate the tumor suppressor proteins p53 and retinoblastoma (RB), respectively, resulting in uncontrolled cellular proliferation. The second pathway is generally HPV-independent and is associated with chronic inflammatory changes; these tumors typically retain p53 function but demonstrate alterations involving cyclin-dependent kinase inhibitor 2A (p16), ultimately promoting abnormal cell-cycle progression and malignant transformation.

Diagnosis of vulvar cancer begins with a thorough examination of the vulva and assessment of suspicious lesions, including their size, location, appearance, and potential involvement of adjacent structures and lymph nodes. Persistent vulvar symptoms or abnormal lesions warrant tissue evaluation, with biopsy and histopathological examination being essential for confirming malignancy and determining the histological subtype. Imaging studies may subsequently be performed to assess local tumor extension, lymph-node involvement, and distant disease and to support staging and treatment planning.

Treatment of vulvar cancer is primarily determined by tumor histology, size, depth of invasion, lymph-node status, and overall stage. Surgery remains the mainstay for localized disease and may involve wide local excision or vulvectomy with assessment of regional lymph nodes, with the goal of achieving complete tumor removal with negative margins. Radiation therapy, with or without concurrent chemotherapy, is commonly incorporated for locally advanced disease or selected patients with adverse pathological features, while systemic therapy may be considered for recurrent or metastatic disease. Management is generally individualized through a multidisciplinary approach.
