This process triggers apoptosis of retinal ganglion cells, thinning of the retinal nerve fiber layer, optic disc cupping, and progressive visual field defects.

 

Most patients with open-angle glaucoma remain asymptomatic until peripheral vision loss becomes evident, whereas acute angle-closure glaucoma presents with sudden severe eye pain, blurred vision, halos around lights, headache, nausea, vomiting, and a red eye, requiring immediate medical attention. Early diagnosis relies on comprehensive ophthalmic evaluation, including tonometry to measure IOP, gonioscopy to assess the drainage angle, optic nerve examination, optical coherence tomography (OCT) for retinal nerve fiber layer analysis, pachymetry to measure corneal thickness, and automated visual field testing.

 

The primary goal of glaucoma treatment is to prevent or slow disease progression by lowering intraocular pressure and preserving visual function, as existing vision loss cannot be reversed. First-line therapy typically consists of topical ocular hypotensive medications, including prostaglandin analogs, ß-blockers, a-adrenergic agonists, carbonic anhydrase inhibitors, and Rho kinase inhibitors, used alone or in combination depending on disease severity. Laser procedures such as selective laser trabeculoplasty (SLT) and laser peripheral iridotomy are effective options for open-angle and angle-closure glaucoma, respectively. Patients with uncontrolled disease may require surgical interventions, including trabeculectomy, glaucoma drainage devices, or minimally invasive glaucoma surgery (MIGS). Lifelong monitoring with regular assessment of IOP, optic nerve structure, and visual fields is essential to optimize treatment and minimize irreversible vision loss.
