Hereditary Angioedema: Understanding

Hereditary Angioedema: Overview

Hereditary angioedema (HAE) is a rare, autosomal dominant genetic disorder characterized by recurrent episodes of non-pruritic, non-pitting subcutaneous and submucosal edema involving the skin, gastrointestinal tract, and upper airway. Most cases are caused by pathogenic variants in the SERPING1 gene, resulting in deficiency (type I) or dysfunction (type II) of C1 esterase inhibitor (C1-INH), while HAE with normal C1-INH is associated with mutations in genes such as F12, PLG, ANGPT1, KNG1, MYOF, or HS3ST6. Attacks may be triggered by physical trauma, emotional stress, infections, hormonal changes, or medical and dental procedures.

Clinical manifestations include recurrent swelling of the extremities, face, genitalia, severe abdominal pain with nausea and vomiting due to gastrointestinal edema, and potentially life-threatening laryngeal edema causing airway obstruction. Because HAE is bradykinin-mediated rather than histamine-mediated, it does not respond to antihistamines, corticosteroids, or epinephrine.

The pathophysiology of HAE centers on uncontrolled activation of the plasma kallikrein–kinin system, resulting from inadequate functional C1-INH. Loss of C1-INH activity permits excessive activation of factor XII and plasma kallikrein, leading to increased production of bradykinin, which binds to bradykinin B2 receptors on vascular endothelial cells and causes increased vascular permeability and localized edema.

Diagnosis is based on a compatible clinical history, family history, and laboratory confirmation demonstrating low C4 levels together with reduced quantitative or functional C1-INH levels in types I and II HAE. Genetic testing may be performed to identify SERPING1 or other disease-associated variants, particularly in patients with normal C1-INH or uncertain diagnoses. Early diagnosis is essential because delayed recognition may increase the risk of recurrent severe attacks and fatal airway compromise.

Management of HAE includes on-demand treatment of acute attacks, short-term prophylaxis before high-risk procedures, and long-term prophylaxis for patients with frequent or severe attacks. Acute attacks are treated with plasma-derived or recombinant C1-INH concentrates, the bradykinin B2 receptor antagonist icatibant, or the plasma kallikrein inhibitor ecallantide, depending on regional availability. Long-term prophylaxis includes subcutaneous C1-INH replacement, the monoclonal antibody lanadelumab, and the oral plasma kallikrein inhibitor berotralstat, all of which significantly reduce attack frequency. Patients should receive individualized treatment plans, education regarding early recognition of laryngeal attacks, access to emergency therapy, and genetic counseling, while emerging gene therapies and RNA-targeted approaches are being investigated as potential disease-modifying treatments.

Hereditary Angioedema Emerging Drugs

ADX-324: ADARx Pharmaceuticals

ADX-324 (onvuzosiran) is an investigational, long-acting small interfering RNA (siRNA) therapeutic being developed by ADARx Pharmaceuticals for the prophylactic treatment of hereditary angioedema (HAE) types I and II. It works by selectively silencing prekallikrein (PKK) production in the liver, thereby reducing bradykinin generation, the key mediator of HAE attacks. ADX-324 is designed for infrequent subcutaneous dosing with the potential for twice-yearly administration, aiming to provide durable prevention of HAE attacks. The candidate is currently being evaluated in the Phase III STOP-HAE randomized, placebo-controlled trial to assess its efficacy, safety, pharmacokinetics, pharmacodynamics, and quality-of-life outcomes.
