The condition may be primary (genetic), resulting from inherited disorders of lipoprotein metabolism, or secondary to obesity, type 2 diabetes mellitus, metabolic syndrome, excessive alcohol intake, chronic kidney disease, hypothyroidism, pregnancy, or medications such as corticosteroids and estrogens. Mild to moderate hypertriglyceridemia is often asymptomatic but is associated with an increased risk of atherosclerotic cardiovascular disease (ASCVD). Patients with markedly elevated triglyceride levels may develop eruptive xanthomas, lipemia retinalis, hepatosplenomegaly, or recurrent episodes of acute pancreatitis characterized by severe abdominal pain, nausea, and vomiting. Hypertriglyceridemia is a common metabolic disorder requiring early recognition and comprehensive cardiovascular risk management.
The pathophysiology of hypertriglyceridemia involves increased production and/or impaired clearance of triglyceride-rich lipoproteins, including very-low-density lipoproteins (VLDL) and chylomicrons. Genetic variants affecting lipoprotein lipase (LPL), APOC2, APOA5, GPIHBP1, LMF1, or APOE, together with insulin resistance, obesity, and hepatic overproduction of VLDL, contribute to triglyceride accumulation in the circulation. Elevated triglyceride-rich lipoprotein remnants promote endothelial dysfunction, vascular inflammation, and atherogenesis, while extreme triglyceride elevations increase pancreatic capillary injury through release of toxic free fatty acids, predisposing to acute pancreatitis.
Diagnosis is based on a fasting lipid profile, supported by evaluation for secondary causes through assessment of glucose metabolism, thyroid function, liver and renal function, medication history, and family history. Additional investigations, including apolipoprotein B measurement or genetic testing, may be considered in selected patients with suspected inherited lipid disorders.
Management of hypertriglyceridemia focuses on reducing triglyceride levels, preventing pancreatitis, and lowering cardiovascular risk through lifestyle modification and pharmacological therapy.
