Obesity is a chronic, relapsing, adiposity-based disease characterized by excessive or abnormal body fat that can impair physical health and increase morbidity and mortality. Traditionally, obesity in adults is defined using BMI ≥30 kg/m², although newer frameworks emphasize that BMI alone does not capture fat distribution or obesity-related organ dysfunction. Central adiposity, excess body fat and associated complications are increasingly incorporated into diagnosis.

Common signs and symptoms include excess body weight, increased waist circumference, breathlessness, fatigue, reduced physical mobility, joint discomfort and sleep disturbances. Obesity is multifactorial and results from interactions among genetic susceptibility, energy imbalance, dietary patterns, physical inactivity, environmental factors, medications, socioeconomic conditions and neuroendocrine regulation.

Pathophysiologically, obesity develops when energy intake chronically exceeds energy expenditure, resulting in adipocyte enlargement and expansion of adipose tissue. Excess adiposity can promote chronic low-grade inflammation, insulin resistance, altered adipokine signaling and metabolic dysfunction, contributing to type 2 diabetes, cardiovascular disease, fatty liver disease, sleep apnoea and musculoskeletal disorders.

Diagnosis of obesity begins with assessment of body mass index (BMI), with a BMI ≥30 kg/m² generally classified as obesity in adults; however, BMI alone does not accurately reflect body-fat distribution or health risk. Waist circumference is therefore often measured to identify central or visceral adiposity, while body-composition techniques such as DXA or bioelectrical impedance may be used when additional assessment is required.

Clinical evaluation includes medical and family history, weight trajectory, diet, physical activity, medications, sleep, and potential secondary causes such as endocrine disorders. Clinicians also assess obesity-related complications through blood pressure measurement and laboratory tests such as glucose/HbA1c, lipid profile and liver-function tests. Increasingly, diagnosis emphasizes the amount and distribution of excess adiposity together with its impact on organ function and overall health, rather than relying on BMI alone.

Treatment is individualized and aims not only to reduce weight but also to improve obesity-related complications and overall health. Lifestyle management, including nutritional intervention, physical activity and behavioral support, remains foundational; however, long-term physiological adaptations can make sustained weight loss difficult. For selected patients with severe or complicated obesity, metabolic/bariatric surgery provides the most durable weight-loss option.
