Systemic Sclerosis Epidemiology

  • Systemic sclerosis affects approximately 100,000 people in the United States, representing about one-third of individuals with scleroderma. It is a chronic, multisystem disease requiring ongoing rheumatologic care, often with input from other medical specialties.
  • Systemic sclerosis has a highly variable presentation, ranging from limited to diffuse disease with potentially severe lung, vascular, gastrointestinal, and renal complications; early symptoms such as Raynaud phenomenon and puffy fingers may precede diagnosis.
  • Systemic sclerosis is associated with high autoantibody prevalence, with antinuclear antibodies present in ≥90% of cases and disease-specific anticentromere, anti-RNA polymerase III, and anti-Scl-70 antibodies in up to 70%; genetic and environmental factors may contribute to disease development.
  • The disease predominantly affects females, with women accounting for approximately 80% of patients, while disease severity and organ involvement can vary according to sex, age, disease subtype, and autoantibody profile.
  • Diagnosis of systemic sclerosis requires a multidisciplinary assessment of clinical features, disease-specific autoantibodies, nailfold capillaroscopy, and internal-organ involvement, as early and heterogeneous presentations can lead to underdiagnosis and treatment delays.

Systemic Sclerosis Understanding and Diagnosis Algorithm

Systemic Sclerosis Overview and Diagnosis

Systemic Sclerosis is a chronic, multisystem autoimmune disorder characterized by immune dysregulation, small-vessel vasculopathy, and progressive fibrosis of the skin and internal organs. The disease is clinically heterogeneous and broadly classified into limited cutaneous and diffuse cutaneous systemic sclerosis, with manifestations ranging from Raynaud phenomenon and skin thickening to severe interstitial lung disease, pulmonary arterial hypertension, gastrointestinal involvement, and scleroderma renal crisis. Disease development is associated with genetic susceptibility, environmental triggers, vascular injury, and dysregulated immune and fibrotic pathways.

Further details are provided in the report.

Systemic Sclerosis Diagnosis

The diagnosis of systemic sclerosis is based on a comprehensive clinical assessment supported by disease-specific autoantibody testing, nailfold capillaroscopy, and evaluation of internal-organ involvement, as no single diagnostic test definitively confirms the disease. The 2013 American College of Rheumatology/European Alliance of Associations for Rheumatology (ACR/EULAR) classification criteria incorporate skin thickening, fingertip lesions, abnormal nailfold capillaries, pulmonary arterial hypertension or interstitial lung disease, Raynaud phenomenon, and Systemic Sclerosis-associated autoantibodies. Anticentromere, anti-topoisomerase I, and anti-RNA polymerase III antibodies provide important diagnostic and prognostic information, while pulmonary function testing, high-resolution computed tomography, echocardiography, and other organ assessments help identify clinically significant internal-organ involvement.

Systemic Sclerosis Epidemiology

Key Findings from Systemic Sclerosis Epidemiological Analysis and Forecast

  • As per DelveInsight's analyst, the diagnosed prevalent cases of systemic sclerosis in the 7MM were estimated to be ~ 232,400 cases in 2025.
  • Among EU4 and the UK, Italy had the highest number (~8%) of diagnosed prevalent cases of systemic sclerosis, while Spain had the lowest number of diagnosed prevalent cases.
  • Among the age-specific cases, 40–69 age group (~60%) had the highest prevalent cases in the US followed by 70 and above in 2025.
  • In the US, mild organ damage cases of systemic sclerosis was found to be more as compared to moderate and severe organ damage cases in 2025.
  • According to DelveInsight estimates, the majority of systemic sclerosis cases in the US are classified as limited cutaneous systemic sclerosis, accounting for approximately 65% of the total cases in 2025, making it the most prevalent subtype compared to diffuse cutaneous systemic sclerosis and systemic sclerosis sine scleroderma.
  • In EU4 and the UK had nearly 81,000 cases of systemic sclerosis in 2025.
  • The prevalence of Systemic Sclerosis is estimated at approximately 10–30 cases per 100,000 individuals worldwide, although reported estimates vary substantially across countries because of differences in diagnostic criteria, case ascertainment, and healthcare access.

Scope of the Report
