Nitric-oxide mediated vasodilators like nitroglycerin and nitroprusside effectively lower blood pressure, but the associated risk of reflex tachycardia should be recognized. The majority of published research regarding the treatment of hyperadrenergic symptoms from cocaine involves using β-adrenoceptor blocking drugs (β-blockers) in human trials and case reports. These studies do not support the use of the drug in event of intoxication.

But the mixed β/α blocker, labetalol, is safe and effective for treating concomitant cocaine-induced hypertension and tachycardia without any “unopposed alpha-stimulation” adverse event. The use of labetalol for the treatment of cocaine-associated chest pain is designated as class IIb-C in the 2013 ACC/AHA guideline.

Agitation and psychosis may occur concomitantly in patients with hyperadrenergic symptoms from acute cocaine toxicity. Dopaminergic receptor antagonist’s haloperidol and droperidol (butyrophenones) and later generation agent’s ziprasidone, olanzapine, risperidone, and aripiprazole are most commonly used for the control of agitation and psychosis.
