Hypoparathyroidism: Understanding

Hypoparathyroidism: Overview

Hypoparathyroidism is an uncommon endocrine disorder characterized by dysfunction of the parathyroid glands, resulting in insufficient secretion of parathyroid hormone (PTH). Because PTH plays a key role in maintaining calcium and phosphate homeostasis, its deficiency leads to hypocalcemia and hyperphosphatemia, which can cause increased neuromuscular irritability. Patients may experience muscle cramps, spasms, twitching, paresthesia, seizures, and, in severe cases, tetany. The condition most commonly occurs following unintended removal or injury of the parathyroid glands during neck surgery, particularly thyroidectomy, although genetic, autoimmune, infiltrative, and other causes.

Hypoparathyroidism results from inadequate secretion or action of parathyroid hormone (PTH), which normally maintains calcium and phosphate balance in the body. Reduced PTH decreases calcium reabsorption in the kidneys, lowers the activation of vitamin D (calcitriol), and reduces calcium mobilization from bone, ultimately resulting in hypocalcemia. At the same time, reduced PTH decreases renal phosphate excretion, causing hyperphosphatemia. The combination of low calcium and high phosphate increases neuromuscular excitability and accounts for many of the clinical manifestations of hypoparathyroidism, including paresthesia, muscle cramps, spasms, and tetany.

The signs and symptoms of hypoparathyroidism are primarily related to hypocalcemia and increased neuromuscular excitability caused by inadequate parathyroid hormone (PTH) secretion. Patients may experience numbness and tingling (paresthesia), muscle cramps, muscle stiffness, twitching, and spasms, particularly involving the hands, feet, and face. More severe hypocalcemia can lead to tetany, seizures, laryngospasm, and cardiac abnormalities, including prolonged QT intervals and arrhythmias. Chronic hypoparathyroidism may also be associated with fatigue, anxiety, depression, cognitive difficulties, headaches, and visual disturbances due to cataracts. The severity of symptoms varies depending on the degree and duration of hypocalcemia.

Hypoparathyroidism is diagnosed primarily through biochemical evaluation, showing low serum calcium and inappropriately low or normal PTH levels. Additional findings may include elevated serum phosphate and low or normal magnesium levels. Diagnosis is supported by repeated measurements of calcium and PTH, while other laboratory tests, such as 25-hydroxyvitamin D, renal function, and urinary calcium, may help identify the underlying cause and assess complications.

Treatment of hypoparathyroidism primarily aims to maintain serum calcium levels and reduce symptoms of hypocalcemia. Conventional therapy includes oral calcium supplements and active vitamin D analogs, such as calcitriol, to improve calcium absorption. Magnesium deficiency should also be corrected, while thiazide diuretics may be used in selected patients with hypercalciuria. In patients who remain inadequately controlled with conventional therapy, parathyroid hormone replacement therapy may be considered.

It also helps to understand clinical trial details, expressive pharmacological action, agreements and collaborations, and the latest news and press releases.

Hypoparathyroidism Emerging Drugs

  • Canvuparatide: MBX Biosciences

Canvuparatide (MBX 2109) is an investigational new drug (delivered via sub-cutaneous injection), that is designed as a potential long-acting hormone replacement therapy for the treatment of chronic hypoparathyroidism, or HP. Leveraging our proprietary PEP platform, we designed canvuparatide to treat the underlying pathophysiology of HP by providing a continuous, infusion-like exposure to parathyroid hormone, or PTH, with convenient once-weekly administration. Currently, the drug is in Phase III stage of its development for the treatment of Hypoparathyroidism.

  • EXT608: Extend Biosciences

EXT608 is an investigational, long-acting parathyroid hormone (PTH) replacement therapy which is a modified form of PTH(1-34) designed to provide sustained activation of the PTH1 receptor (PTH1R) while extending the peptide’s circulation time through Extend Biosciences’ D-VITylation® technology, which attaches a vitamin D derivative to PTH and promotes reversible binding to vitamin D-binding protein, thereby reducing renal clearance. By activating PTH1R, EXT608 is intended to reproduce the physiological effects of endogenous PTH, including increasing renal calcium reabsorption, stimulating conversion of vitamin D to active calcitriol, enhancing calcium mobilization from bone, and increasing renal phosphate excretion. Currently, the drug is in Phase II stage of its development for the treatment of Hypoparathyroidism.
