Von Willebrand’s Disease: Understanding

Von Willebrand’s Disease: Overview

Von Willebrand disease (VWD) is a common inherited or acquired bleeding disorder caused by a quantitative deficiency or qualitative defect of von Willebrand factor (VWF), a glycoprotein essential for normal hemostasis. VWF is produced primarily by endothelial cells and megakaryocytes and functions both as a mediator of platelet adhesion at sites of vascular injury and as a carrier that stabilizes coagulation factor VIII. Deficiency or dysfunction of VWF therefore results in impaired primary hemostasis and, in some cases, reduced factor VIII levels, leading to an increased tendency for bleeding.

VWD may be inherited or acquired. Inherited VWD is classified into three major types: Type 1, the most common form, results from a partial quantitative deficiency of VWF; Type 2 results from qualitative defects in VWF and includes subtypes 2A, 2B, 2M, and 2N; and Type 3 is the most severe form, characterized by an almost complete or complete absence of VWF and an autosomal recessive inheritance pattern. Acquired VWD can develop secondary to conditions such as malignancies, lymphoproliferative disorders, autoimmune diseases, hypothyroidism, myeloproliferative neoplasms, and high-flow cardiovascular states.

VWF is a large multimeric glycoprotein synthesized by endothelial cells and megakaryocytes. Following vascular injury, VWF facilitates platelet adhesion to the damaged vessel wall, thereby contributing to primary hemostasis. Larger VWF multimers have greater hemostatic activity and provide more binding sites for platelets and endothelial components. VWF also protects factor VIII from degradation, thereby maintaining its circulating levels. In VWD, reduced VWF quantity or impaired VWF function compromises platelet adhesion and may increase factor VIII degradation, resulting in prolonged bleeding. The specific defect and severity vary according to the VWD subtype.

Diagnosis of VWD is based on the patient's bleeding history together with laboratory evaluation. Common clinical manifestations include recurrent or excessive bruising, prolonged bleeding following minor trauma or dental procedures, epistaxis, and heavy menstrual bleeding. Initial laboratory assessment includes a complete blood count and coagulation profile; platelet counts are generally normal, prothrombin time (PT) is usually normal, while activated partial thromboplastin time (aPTT) may be prolonged because of reduced factor VIII levels.

Treatment depends on the VWD subtype, disease severity, bleeding episode, and clinical setting. Desmopressin (DDAVP) is commonly used in Type 1 VWD and selected Type 2 subtypes because it stimulates the release of stored VWF from endothelial cells, thereby increasing VWF and factor VIII levels. Patients with Type 3 VWD or severe VWD who do not respond adequately to DDAVP may require VWF replacement therapy, including VWF-containing factor VIII concentrates, particularly during major bleeding, trauma, or surgery. Tranexamic acid, an antifibrinolytic agent, may be used to control mucosal bleeding by reducing fibrin clot breakdown.

Von Willebrand’s Disease Emerging Drugs

  • VGA039: Incyte

VGA039 (latarcibart) is an investigational monoclonal antibody therapy with a novel mechanism of action that targets Protein S, with dual actions promoting platelet attachment and enhancing fibrin deposition to restore hemostasis. Latarcibart has the potential to be a universal prophylactic therapy for numerous bleeding disorders, starting with all types of von Willebrand disease (VWD) and bleeding sites. As a subcutaneously self-administered investigational antibody therapy with a once monthly dosing regimen, latarcibart has the potential to improve bleeding outcomes, convenience, and quality of life for patients. Currently, the drug is being evaluated in the Phase III stage of its development for the treatment of Von Willebrand’s Disease.
