Myocardial Infarction: Understanding
Myocardial Infarction: Overview
Myocardial infarction (MI), commonly known as a heart attack, is an acute coronary syndrome caused by prolonged interruption of myocardial blood flow, resulting in ischemic injury and irreversible necrosis of cardiac muscle. The majority of MIs occur due to rupture or erosion of an atherosclerotic plaque followed by thrombus formation within a coronary artery. Risk factors include hypertension, dyslipidemia, diabetes mellitus, smoking, obesity, physical inactivity, advanced age, and family history of premature cardiovascular disease. Patients typically present with severe, persistent substernal chest pain or pressure radiating to the left arm, shoulder, neck, jaw, or back, accompanied by dyspnea, diaphoresis, nausea, vomiting, dizziness, and fatigue; however, atypical or silent presentations are common in women, older adults, and patients with diabetes.
Myocardial infarction remains a leading cause of morbidity and mortality worldwide despite advances in prevention and acute cardiovascular care.
The pathophysiology of myocardial infarction begins with atherosclerotic plaque rupture or erosion, exposing thrombogenic material that activates platelets and the coagulation cascade, leading to formation of an occlusive coronary thrombus. Abrupt cessation of coronary blood flow causes myocardial ischemia, cellular energy depletion, calcium overload, oxidative stress, and progressive cardiomyocyte death if reperfusion is not rapidly achieved. The extent of myocardial damage depends on the duration of ischemia, collateral circulation, and the affected coronary territory.
Diagnosis is established using the Fourth Universal Definition of Myocardial Infarction, requiring evidence of myocardial injury demonstrated by a rise and/or fall in cardiac troponin with at least one value above the 99th percentile, together with clinical evidence of acute myocardial ischemia. Diagnostic evaluation includes electrocardiography (ECG) to distinguish ST-segment elevation myocardial infarction (STEMI) from non-ST-segment elevation myocardial infarction (NSTEMI), echocardiography, coronary angiography, and selected use of cardiac magnetic resonance imaging.
Management of myocardial infarction focuses on rapid restoration of coronary blood flow, limiting infarct size, preventing complications, and reducing long-term cardiovascular risk. Primary percutaneous coronary intervention (PCI) is the preferred reperfusion strategy for STEMI when performed promptly, while fibrinolytic therapy is considered when timely PCI is unavailable. Guideline-directed medical therapy includes dual antiplatelet therapy (aspirin plus a P2Y12 inhibitor), anticoagulation, high-intensity statins, ?-blockers, ACE inhibitors or ARBs, and mineralocorticoid receptor antagonists for appropriate patients with left ventricular dysfunction. Long-term management includes cardiac rehabilitation, smoking cessation, heart-healthy dietary modification, regular physical activity, control of hypertension, diabetes, and dyslipidemia, and adherence to secondary prevention therapies to reduce recurrent cardiovascular events and improve survival.

Myocardial Infarction Emerging Drugs Chapters
This segment of the Myocardial Infarction report encloses its detailed analysis of various drugs in different stages of clinical development, including Phase III, II, I, Preclinical and Discovery. It also helps to understand clinical trial details, expressive pharmacological action, agreements and collaborations, and the latest news and press releases.
Myocardial Infarction Emerging Drugs
Zalunfiban: Celecor Therapeutics
Zalunfiban (Disaggpro™; formerly RUC-4) is an investigational subcutaneous glycoprotein IIb/IIIa (integrin ?IIb?3) receptor inhibitor designed for rapid administration at the first point of medical contact in patients with ST-segment elevation myocardial infarction (STEMI). It provides potent platelet inhibition within approximately 15 minutes, helping to restore coronary blood flow before primary percutaneous coronary intervention (PCI) and limiting ischemic myocardial damage. The drug was specifically developed for pre-hospital use by emergency medical personnel and has a short duration of action, allowing rapid recovery of platelet function and potentially reducing bleeding risk. Following positive Phase III CELEBRATE trial results, CeleCor Therapeutics completed submission of a New Drug Application (NDA) to the US FDA in June 2026; the drug has received FDA Fast Track designation and Rolling Review.
TQJ230: Novartis
TQJ230 (pelacarsen) is an investigational antisense oligonucleotide designed to selectively reduce hepatic production of apolipoprotein(a), thereby lowering circulating lipoprotein(a) [Lp(a)], an independent genetic risk factor for cardiovascular disease. It is being evaluated in patients with a history of myocardial infarction and other established atherosclerotic cardiovascular diseases who have elevated Lp(a) levels despite standard therapy. The drug is administered as a once-monthly 80 mg subcutaneous injection and aims to reduce the risk of major adverse cardiovascular events, including recurrent myocardial infarction, stroke, cardiovascular death, and urgent coronary revascularization. It is currently being assessed in the global Phase III Lp(a)HORIZON trial sponsored by Novartis.
NL005: Beijing Northland Biotech. Co., Ltd.
NL005 is an investigational recombinant human thymosin beta 4 (T?4) injection being developed by Beijing Northland Biotech for the treatment of acute ST-segment elevation myocardial infarction (STEMI) following primary percutaneous coronary intervention (PCI). Thymosin beta 4 is a naturally occurring peptide with cardioprotective, anti-inflammatory, pro-angiogenic, and tissue repair properties, designed to reduce ischemia-reperfusion injury and promote myocardial regeneration. The drug is administered intravenously within 4 hours after PCI, followed by once-daily dosing for 7 days, with the aim of reducing infarct size, preserving left ventricular function, and improving long-term cardiac recovery. It is currently being evaluated in a randomized, double-blind, placebo-controlled Phase IIc clinical trial assessing its efficacy, safety, and pharmacokinetics in patients with acute myocardial infarction.
