Sanfilippo Syndrome: Understanding

Sanfilippo Syndrome: Overview

Sanfilippo syndrome, also known as mucopolysaccharidosis type III (MPS III), is a rare autosomal recessive lysosomal storage disorder characterized by progressive neurodegeneration resulting from impaired degradation of heparan sulfate. The disease is classified into four subtypes (MPS IIIA, IIIB, IIIC, and IIID), each caused by deficiency of a different lysosomal enzyme involved in heparan sulfate catabolism. Unlike other mucopolysaccharidoses, Sanfilippo syndrome predominantly affects the central nervous system, leading to progressive cognitive decline, behavioral disturbances, loss of motor function, and reduced life expectancy, while somatic manifestations are generally milder.

Sanfilippo syndrome is caused by biallelic pathogenic variants in genes encoding lysosomal enzymes responsible for the sequential degradation of heparan sulfate. Deficiency of heparan N-sulfatase (SGSH) causes MPS IIIA, ?-N-acetylglucosaminidase (NAGLU) deficiency results in MPS IIIB, acetyl-CoA:?-glucosaminide N-acetyltransferase (HGSNAT) deficiency leads to MPS IIIC, and N-acetylglucosamine-6-sulfatase (GNS) deficiency causes MPS IIID. The absence or marked reduction of these enzymes prevents normal lysosomal breakdown of heparan sulfate, resulting in its progressive accumulation within cells, particularly neurons of the central nervous system.

Sanfilippo syndrome results from the lysosomal accumulation of heparan sulfate due to deficiency of enzymes involved in its degradation. Progressive substrate storage disrupts lysosomal function, triggers neuroinflammation and neuronal dysfunction, and ultimately leads to cognitive decline, behavioral abnormalities, and progressive neurodegeneration, with the central nervous system being the primary site of disease involvement.

Diagnosis of Sanfilippo syndrome is based on clinical assessment, detection of elevated urinary heparan sulfate, and confirmation of reduced activity of the affected lysosomal enzyme in leukocytes or fibroblasts. Molecular genetic testing is subsequently performed to identify the causative gene mutation and confirm the disease subtype.

Currently, there is no approved curative therapy for Sanfilippo syndrome, and management is primarily supportive and multidisciplinary. Treatment focuses on controlling behavioral disturbances, seizures, sleep disorders, mobility limitations, and other disease-related complications through pharmacological interventions, physical and occupational therapy, speech therapy, and nutritional support. Research efforts are actively investigating enzyme replacement therapy, gene therapy, substrate reduction therapy, stem cell-based approaches, and pharmacological chaperones with the goal of slowing disease progression and preserving neurological function.

It also helps to understand clinical trial details, expressive pharmacological action, agreements and collaborations, and the latest news and press releases.

Sanfilippo Syndrome Emerging Drugs

  • JR-446: JCR Pharmaceuticals

JR-446 is an investigational enzyme replacement therapy which applies JCR's proprietary J-Brain Cargo® technology, which is designed to deliver therapeutic enzymes across the blood–brain barrier via transferrin receptor-mediated transport. JR-446 consists of a recombinant ?-N-acetylglucosaminidase (NAGLU) enzyme fused with J-Brain Cargo®, enabling delivery of the enzyme to the central nervous system. By supplementing the deficient NAGLU enzyme, JR-446 is intended to reduce the accumulation of heparan sulfate, the substrate that accumulates in patients with MPS IIIB due to NAGLU deficiency. Currently, the drug is being evaluated in the Phase I/II stage of its development for the treatment of Sanfilippo Syndrome.

  • GC1130A: GC Biopharma Corp./ Novel Pharma

GC1130A is an investigational intracerebroventricular (ICV) enzyme replacement therapy (ERT) being developed by GC Biopharma Corp. in collaboration with Novel Pharma for the treatment of Sanfilippo syndrome type A (MPS IIIA). The therapy consists of recombinant human heparan N-sulfatase (rhHNS) and is administered directly into the cerebral ventricles via an intracerebroventricular access device to bypass the blood–brain barrier. GC1130A is intended to replace the deficient heparan N-sulfatase enzyme in patients with MPS IIIA, thereby reducing the accumulation of heparan sulfate, which causes central nervous system damage and progressive neurodegeneration. Currently, the drug is being evaluated in the Phase I stage of its development for the treatment of Sanfilippo Syndrome.
